Source: Asbury Park Press, N.文件倉J.Sept. 21--Four nursing homes in Ocean and Monmouth counties have garnered the lowest rating in a national Medicare survey, placing them among 33 facilities in New Jersey that scored that poorly.In Ocean County, they are Manchester Manor Rehabilitation in Manchester, and Rose Garden Nursing and Rehabilitation Center and Shore Meadows Rehabilitation and Nursing Center, both in Toms River. In Monmouth County, there is one: Gateway Care Center in Eatontown. All are for-profit facilities.Two Ocean County nursing homes that received three years of one-star ratings in a USA Today analysis released in February 2012 have since increased their ratings to four out of five stars, according to new Medicare information. The facilities are: Holiday Care Center in Berkeley and Hampton Ridge Healthcare and Rehabilitation in Toms River.There are seven five-star facilities in Ocean County and nine in Monmouth County.The five-star rating system of the federal Centers for Medicare & Medicaid Services (CMS) is based on health inspections, staffing and "quality measures." Quality measures is a nursing home's care of patients from how it "helps people keep their ability to dress and eat, or how well the nursing home prevents and treats skin ulcers," according to Medicare.The Nursing Home Compare web site can be found at .medicare.gov/nursinghomecompare .The federal government contracts with states to inspect nursing homes. The unannounced inspections occur once a year for the facilities that have not received special status because of low ratings."The inspections are rigorous, involved and thorough," Jeffrey Hall, spokesman for CMS, said in an e-mail.The center only certifies skilled nursing facilities, not nursing homes that only provide custodial care, he said.Not everyone agrees on the thoroughness of the inspections."Serious problems are not always identified," said Robyn Grant, director of public policy and advocacy for The National Consumer Voice for Quality Long-Term Care, based in Washington.But she praised the overall process and said that inspectors do a "good job in terms of digging in and getting" details."The survey report is the only objective source out there," she said. "It's an important place for families and people looking into nursing homes to look, but it's a starting point, not a end all and be all."The star ratings are part of a broader federal effort to increase transparency for consumers of health care. When introduced in late 2008, nursing home industry groups called them simplistic and unfair.A summary of the issues Medicare found at the one-star facilities:ROSE GARDEN NURSING AND REHABILITATION CENTERThe 124-bed facility on Old Freehold Road in Toms River is run by Ocean Convalescent Center Inc.Among other shortcomings, the facility failed to display its most recent 1-star survey for residents and visitors to view.According to a May 2012 inspection report, an unspecified incident or incidents occurred in which Rose Garden failed to "protect each resident from all abuse, physical punishment, and being separated from others."The facility was also cited for failing to "hire only people with no legal history of abusing, neglecting or mistreating residents" or failing to "report and investigate any acts or reports of abuse, neglect or mistreatment of residents." No details were provided.The facility has seen one-star overall ratings since 2010. Messages left for Administrator Andrew Shawn were not returned.MANCHESTER MANOR REHABILITATIONThe 165-bed facility on Tobias Avenue in Manchester is run by Lazman Inc.The facility ha存倉 received one star ratings for the past five years, one of seven facilities in New Jersey to garner such a low rating for that time period.During a health inspection in November 2012, the facility failed "to provide care for residents in a way that keeps or builds each resident's dignity and respect of individuality."A resident complained of being left on a bedpan for 20 to 30 minutes."The resident added that she felt thoroughly disgusted and like she didn't exist," according to the report.Manchester was also cited for an active water leak from the ceiling of dietary storage closet in the Main Dining room."The ceiling tile was wet and had a blackish mold-like growth on it," the report read.Inspectors found that the facility served residents cold beef stroganoff, noodles and Italian green beans that the inspector confirmed by tasting.Like all facilities, it is required to "prepare food that is nutritional, appetizing, tasty, attractive, well-cooked, and at the right temperature."The center failed "to develop policies that prevent mistreatment, neglect, or abuse of residents or theft of resident property," according to the inspection report, but the report did not elaborate.Administrator Gary Pizzichillo did not return messages left at the center.SHORE MEADOWS REHABILITATION AND NURSING CENTERThe 149-bed facility on Warner Street in Toms River is run by Regal Operations LLC.The facility was cited for failing to "provide necessary care and services to maintain the highest well being of each resident."In an inspection in December, a resident told inspectors that he or she had to wait up to one to two hours for pain medication after ringing a bell for a staff member."This deficient practice was observed for one of eight residents," the report stated.On patient at the facility was left in bed without a side rail and fell sustaining a right hip fracture, according to the report.During an April inspection, it was determined that the facility failed to "consistently provide incontinent care in a timely manner for one of five sampled residents," according to the inspection report.The facility, however, did receive a three-star rating for quality measures."The facility is in compliance as of the last survey with the state," said Catherine Fitzgerald, regional administrator for Shore Meadows. "The CMS survey is a snippet in time and doesn't really reflect the care the facility gives which is proven by the repeat customers that come for care in the community."GATEWAY CARE CENTERThe 178- bed facility on Grant Avenue in Eatontown also shows a one-star rating on the CMS Nursing Home Compare web site. But Gateway's story is more complicated. It garnered a four-star rating in "quality measures." And it's overall rating appears to be on the upswing. After receiving one-star overall ratings in 2011 and 2012, the facility scored two stars in recent months, not enough to affect its rating for this year, though.A staff member said Barbara Darlington, the facilities administrator, was on vacation and was not reachable. No one else would comment, the staff member said.Among problems at the facility uncovered during an inspection in April, inspectors found that that the facility failed to investigate two reported falls that led to injuries.
